Not really a severe case of bad service, but certainly memorable and worth a good laugh.
A few days ago, I decided to have dinner with my mom at a local restaurant. As is my bad habit, I ordered a margarita. The server asked me the usual questions, and left with an order for the dinners and my margarita on the rocks and without salt. She then came back a minute or so later, with a question that she told me she had forgotten to ask.
She wanted to know if there was any particular brand of vodka I had wanted in my margarita.
